Emotional dysregulation may express differently in borderline personality disorder (BPD), possibly due to the influence of the cultural context. The objective of this study was to characterize the difficulties in emotional regulation in patients with BPD who consult a center specialized in Dialectical Behavioral Therapy (DBT) in Medellín, Colombia.  A descriptive cross-sectional study was carried out with 54 patients, mainly young, single, and high socioeconomic status women, who were admitted to treatment and evaluated with “Difficulties in Emotional Regulation Scale” (DERS). The median (M) and interquartile range (IQR) and the p value were calculated with the U Mann-Whitney with their respective effect size (r value). The total score on the DERS was high (M = 134; IQR = 117-142). Differences were found between men and women, with an intermediate effect size for the total score (Female = 135.5 versus Male = 119; p = 0.047; r = -0.26). No differences were found between age groups. When analyzing by comorbidities, it was found that the difficulties in emotional regulation were different in patients who also had ADHD, anxiety, depression, and anxiety combined, and bipolar disorder. This may indicate that in patients with BPD from a city in Colombia in DBT, the difficulties in emotional regulation are high and seem to be greater in women. The presence of anxiety, depression, ADHD, and bipolar disorder could influence the intensity of emotional dysregulation and the facets in which it is shown, which suggests a high variability within the diagnosis.en-US
