dc.description	According to the idea of “paradigms” in psychiatry, formulated by psychiatrist G. Lantéri-Laura we propose the thesis of the current “paradigm” of psychiatry, which predominates in the nosology, the clinics, the epidemiology, the theory and the treatments of the psychological field. Following the thesis and the presentation of Lantéri-Laura of the first three paradigms he identifies, we formulate, by hypothesis, the existence of a “fourth paradigm”, which is a result of the appearance of the first psychotropic drug, of psychopharmacology and of what we call the “pharmaceutical apparatus.” This overhaul will produce new types of “disorders”: “pharmaco-modified disorders” and “pharmaco-derived disorders” or “new syndromes”. The “pharmaceutical apparatus” and the breach opened by the socio-economical changes will also redesign and allow the emergence of new forms of psychotherapy and a new rationality inspired by the management of risks. The incidence of evidence-based medicine on psychotherapy and the rise in the last twenty years of empirically supported therapy (mainly cognitive behavior therapies) will result in the entry of a school of psychotherapeutic thought that belongs to the “fourth paradigm”.	en-US
dc.description	Es a partir de la idea de “paradigmas” en psiquiatría, formulada por el psiquiatra G. Lanteri-Laura, que proponemos la tesis del paradigma actual de la psiquiatría, éste es el que predomina en la nosología, la epidemiología, la clínica, la teoría y los tratamientos del campo “psi”. Siguiendo la presentación de la tesis de Lanteri-Laura de los tres primeros paradigmas que él identifica, se propone, por hipótesis, la existencia de un “cuarto paradigma”, que es dependiente de la aparición del primer medicamento psicotrópico, la psicofarmacología y de lo que llamamos el “dispositivo farmacéutico”. Este reajuste producirá nuevas concepciones de los “trastornos”: “trastornos fármaco-modificados” y “trastornos fármaco-deducidos” o “nuevos síndromes”. El dispositivo farmacéutico y la brecha abierta por los cambios socio-económicos van también rediseñar y permitir el surgimiento de nuevas formas de psicoterapia y van a inspirar una nueva racionalidad en la gestión de riesgos. La incidencia de la medicina de pruebas sobre la psicoterapia y el nacimiento en los últimos veinte años de las terapias empíricamente sustentadas (terapias cognitivo-conductuales en su mayoría), darán lugar a la entrada de una corriente de pensamiento psicoterapéutico específica al cuarto paradigma.	es-ES
